Guided surgery removes the guesswork when deciding which areas of the jawbone are suitable for dental implant procedures. The process involves CBCT scanning and digital intraoral impressions to create a 3-D image of your jaw and oral anatomy. This will provide accurate information on the amount and location of available bone. A digital treatment plan is developed to help determine the precise location of each implant before the surgery. Then a surgical guide is produced, which will help control the osteotomy (or the surgical removal of bone), in unison with the predetermined implant placement. This allows us to guide in titanium sleeves that will eventually house the implants, according to the precise depth and angle predetermined in your treatment plan.

Thanks to CBCT scans, we can also create the permanent restoration before surgery. In some cases, we will even be able to place it the same day as surgery, simplifying your treatment process and improving your overall satisfaction.

Freehand Surgery vs. Guided Surgery?

Freehand surgery cases, use periapical and panoramic radiographs to determine the bone available for implant placement. Simply put, we rely on X-rays and an intraoral examination to give us a reasonable idea of the bone underneath your gums. We use surrounding teeth as guides, and fabricate a study cast to better understand the measurements of the space available. We can also create a surgical stent and us it as a tool for evaluation at the time of surgery. It is not until the actual surgical procedure that the underlying bone can be directly evaluated and measured with calipers to precise place the implant. This not a major concern, but it is something to consider, especially if resorbed bone is an issue.

This simplifies the task of situating an implant within the available bone. It offers a predictability that is otherwise unachievable with freehand surgery. You will know exactly how your new implant or bridge will look, before restoring your smile. This is why guided surgery is considered to be prosthetically driven, with final esthetics drastically improved in recent years.

The AAOMR (American Academy of Oral and Maxillofacial Radiology), recommends CBCT above standard radiographs, for obtaining accurate and comprehensive information on all implant sites. With 3-D imaging, we can see where a nerve lies and determine bone density. We can decide on optimal placement of the implant platform and approximate how the implant screw will access the final restoration. Moreover, of course, we can predetermine the accurate width and depth of the proposed implant.

Guided surgery comes at a higher cost than freehand surgery, because of expenses associated with the CBCT scanning equipment. However, the cost is worth it if you want precise and aesthetically pleasing implants with relative ease.
